Photography consent and release form

In accordance with our child protection policy, we will not permit photographs, video or other images of young people to be taken without consent. If the child is under 16, consent must be obtained from a parent / carer.

We would be grateful if you would fill in this form to give us permission to take photos of your child and use these in our printed and online publicity as well our as Drama Club App and performances.

I give permission for the Actors' Institute and Shoebox Theatre to take photographs and / or video of my child in classes and performances.

I grant the full rights to the Actors' Institute / Shoebox Theatre to use the images resulting from the photography/video filming, and any reproductions or adaptations of the images for fundraising, publicity or other purposes to help achieve the organisation's aims. This might include (but is not limited to), the right to use them in their printed and online publicity, social media, press releases, within performances, on the Drama Club App and funding applications.
